The December 2020 edition of Clinical Communiqué features the important issue of clinical handover to primary care. The transition between hospital to primary care is extremely high-risk for many reasons that can include unclear management plans and delays in critical information that could impact treatment. The case studies highlight the adverse outcomes of delayed handover and communication required for patient follow-up. Recommendations such as a standardised process for clinical handover, including educating patients of their condition and clear post-discharge plans communicated to their general practitioners, will ensure the continuation of care for patients.
